Challenge

The Family School Wellness program operates as a joint partnership with 11 funding bodies that each have numerous reporting and funding applications that are required on a quarterly basis. Case Management was a real challenge especially with student mobility, decentralized record keeping, and lack of visibility into what other clinicians were doing. Gathering data and statistics for quarterly reports was time consuming and difficult to quantify. Tracking formal consents was difficult and slowed the process of moving forward with the student and/or their family.

Solution

30 clinicians working seamlessly within a modern case management system, that is perfectly aligned with program needs and requirements. Clinicians are able to easily capture contact notes, demographic information, assessment data etc. Providing clinical supervision and oversight is now easier and has increased accountability for wellness workers. Access to student records has ensured continuity as students move throughout the school division, from one school to another school. Data is more seamless and available to funders and stakeholders.
